Warranty Service. To obtain warranty service, contact the TACOM Target Acquisition Team Item Manager
at (309) 782-5578 (DSN 793-5578). Complete a DA Form 2407, Maintenance Request. Refer to the
following table and DA PAM 738-750 as a guide in completing DA Form 2407. The Item Manager will
provide a number for which to FAX the DA Form 2407.

Neither the government nor the contractor will track other than the IPADS end item serial
number.

Disposition. The TACOM Target Acquisition Team will provide shipping instructions based on the
following scenarios:
(1) Unit within OCONUS. A funded requisition must be provided at the time of the warranty request.
Upon determination by the contractor that the failed LRU is covered under terms of the warranty, the
funded requisition will be cancelled back to the user. The user will be billed for the cost of a
replacement LRU if the failed LRU is not received. This ensures that the unserviceable item will be
returned by the using unit.
